COVID 19 Vaccines | The Palms GP Surgery

We are delighted to be able to continue to vaccinate people aged over 70 years of age. The supply of vaccines has been less than expected and therefore this is taking longer than was anticipated across the country. We will contact you as soon as we have sufficient supply for your age group. There is no need to contact the surgery. The vaccine you will be given here is the Pfizer/Biontech vaccine if you are over 70 years of age.

Patients at very high risk and high risk aged between 16-69 years of age

We are experiencing many enquires from people regarding the vaccine for these groups.

Hospitals have been arranging appointments for these groups. In the next number of weeks we will be provided with vaccine for the very high risk group. We have not received details of deliveries yet.

You should attend either the hospital or the practice depending on who contacts you first

The prioritization is decided by the HSE and not the practice.

The only vaccine available for these groups in the practice is the Astra Zeneca vaccine

We will contact people directly once we have details of vaccine deliveries based on the records we hold on file. However if you wish to notify us that you believe you are eligible based on the HSE list of priority conditions please complete the form below. We will contact you if you are eligible and once we have been notified of vaccine delivery. We are only dealing with the very high risk group at this time. Please identify which of the following very high risk categories you meet. We will review your file to determine eligibility

Obesity (BMI >40kg/m2)
Diabetes with HbA1c >58mmol/mol in the past year
On home oxygen
Awaiting lung transplant or history of lung transplant
Hospital admission due to COPD, cystic fibrosis or lung fibrosis in the past year
All cancer patients actively receiving chemotherapy or radiotherapy for lung or head or neck cancer or who have metastatic cancer
On dialysis or eGFR<15ml/min
Severe immunocompromise due to disease or treatment, for example: transplantation: - Listed for solid organ or haematopoietic stem cell transplant (HSCT) - Post solid organ transplant at any time - Post HSCT within 12 months Treatment: -such as Cyclophosphamide, Rituximab, Alemtuzumab, Cladribine or Ocrelizumab in the last 6 months

This list allows you to make us aware if you believe you are eligible for the very high risk group. If you are we will contact you once we receive sufficient vaccine doses. If the hospital contacts you in advance for a vaccine please attend there for vaccination. Please be aware these are the categories that the HSE have designated. We have no role in deciding what groups get priority
