Please enter as much detail as possible on the form. We aim to have the Prescription issued to the pharmacy within 2 working days of receiving the request. Please note there will be a 15 euro charge for issuing a private prescription.
All prescriptions are now issued directly to your nominated pharmacy and we will no longer be issuing paper prescriptions. You should contact the pharmacy to arrange for collection or delivery of the medicines.
