Chronic Disease Clinics
We run several chronic disease clinics here at the Palms surgery looking after a variety of conditions which include
- Asthma and COPD
- Diabetes
- Chronic Heart Disease
- Hypertension
- Heart Failure
We also have links with St Vincents Hospital and take part in virtual consultation with Heart Failure team to optimise patients management in the community. There is a regular COPD clinic in the practice on a monthly basis as well.
HSE Chronic Disease Management Programme
The HSE rolled out a programme for management of chronic disease in over 75s who have the GMS/DVC. This involves twice yearly reviews with bloods and other tests as determined by your underlying conditions.

The rollout of the scheme has been hampered by COVID outbreak but we will endeavour to have a phased return of the scheme in the coming months.
